Y Community Health Living Index Assessment

Background

This portal is extending the Y Beam website that was started in 2013 by the Y IT taskforce.

It aims to show collaboration potential of the Y's Community Health Index Assessment Initiative
with Rochester area's health related organizations and projects already in flight,
in particular, Olmsted County Community Health Needs Assessment and Improvement.

It's hoped by building out this kind of portal our community becomes more connected as we drive toward health improvements in a systematically.
